In Colombo, on 9 Nov 1921, one K IV crew member is wounded and two others are killed in an explosion in the forward battery compartment. The two casualties are Kwartiermeester L.A.J. Huisman and Kpl. cook J.A. Blom. Kwartiermeester P.A. Melieste is heavily wounded but he recovers.
